1. Click the Oral Exam Form link. You will be routed to DocuSign. You will need to login with your GT Credentials (gtAccount@gatech.edu). 

2. Please verify your name and GT e-mail in the recipient field. Click SEND at the bottom right corner of the page. 

3. Once you click "SEND", you will then be routed to complete and sign the document.

4. When ready to review and sign, you will see the form. Your name should pre-populate, but if it does not, please enter your name in the applicable space. You will also need to input your GTID on the next line.  

5. Please complete the form, and "Sign". Click "FINISH" at the top of the screen to send.  

6. Once complete, a copy of the form will be sent directly to dgs@math.gatech.edu. You will also receive a copy of the form. Please save the attachments since DocuSign will only retain them for six months.
